

Designing a better onboarding experience for an AI symptom checker at clinics
Enabling doctors to spend less time on paperwork and more on patient care.
D4V is a Tokyo-based early stage Venture Capital in partnership with IDEO. Two years ago, D4V invested in healthcare startup Ubie, which develops a service called AI Monshin Ubie. The digital service was created with the goal of reducing the burden on healthcare professionals working long hours, under harsh conditions. AI Monshin Ubie automates a number of patient registration and documentation processes through digital tools, enabling doctors to spend less of their time on manual paperwork, and to dedicate their valuable skills and attention to the patients themselves. Currently, the service is installed in 200 medical institutions in 41 prefectures across Japan.
With AI Monshin Ubie, patients fill out preliminary forms on a tablet, rather than paper, entering information about their health history and current symptoms directly into their digital medical record. As patients fill out the questionnaire, the Ubie AI generates questions in real-time according to patients’ responses, which results in more appropriate and accurate data. The patient’s responses are automatically formatted for the doctor to review prior to and during the consultation. As a result, doctor consultation times and facility operation times are drastically reduced.
With the Coronavirus raising fears of a possible health system breakdown, Ubie has made improvements to anticipate and possibly prevent in-hospital infections. Until recently, Ubie’s services have been primarily used by large hospitals; but in light of the global health crisis, Ubie approached D4V—who partners with IDEO—to help rethink the onboarding experience and quickly facilitate the introduction of AI Monshin Ubie in regional clinics across Japan. D4V and IDEO have been working with Ubie for many years to collaborate in various areas, including UX, rebranding, international research, and more. Therefore, they had a foundation of knowledge from which to build these new features.
Typically, AI Monshin Ubie has been introduced to new hospital clients through direct, in-depth instructional visits from Ubie sales staff. This approach has worked well for rolling out the product to Japan’s 8,000 large hospitals; but for the 100,000 smaller clinics, Ubie needed a more efficient onboarding method to enable staff to get set up smoothly and independently.
During research, D4V and IDEO team members engaged with many different clinic staff members, including full-time doctors, part-time doctors, nurses and receptionists. The researchers found that staff tend to react skeptically to new tools and services, with a common preconception that they will add to their workload. One clinic director’s first impression of AI Monshin Ubie was, “It looks complicated.” But once he actually used the service, he realized that it was very user-friendly.
The team quickly understood that it was important to remove preconceptions by clearly communicating Ubie’s benefits and ease of use. They created a two-minute animation and designed three separate guidebooks. The animation adopted a friendly tone and expressed empathy for doctors, staff members and patients, highlighting how Ubie can bring benefits to each of them. The animation showed how Ubie fits into the existing workflow and explained complex systems in a straightforward and accessible way.

The guidebook for doctors and directors explains how Ubie’s AI smooths the patient consultation experience and greatly improves the quality of medical care. It also introduces concrete how-tos around set-up, implementation, and ideal workflow. FAQs were also included, addressing concerns about whether or not elderly people would be able to use it.

An improved airway intubation system that helps physicians save more lives
The GlideScope Core offers elevated visibility and a seamless workflow for intubation, allowing doctors to better conduct routine procedures and respond to health crises.
In the emergency room, an intervention that’s administered too slowly could cost a patient their life. With COVID-19 fueling a surge in acute respiratory conditions, intubation—a procedure that involves inserting a tube through the mouth or nose and into the airway—is more common than ever. For patients struggling to breathe, the tube can be connected to a ventilator that pumps oxygen to the lungs, keeping them alive.
While a laser-focus on the present is critical for successful patient outcomes, hospitals and physicians often don’t have time to look ahead and anticipate the future of the operating room experience. As technology rapidly improves, there is a ripe opportunity and pressing need to evolve medical equipment to be more innovative and intuitive. Verathon Medical, a global medical device manufacturer, is supporting health care providers at the cutting-edge of care.
Verathon approached IDEO to redesign the existing product system for laryngoscopies and bronchoscopies—procedures that examine parts of the throat through intubation. During these examinations, a tube with a camera embedded in the tip is inserted through the patient’s nose or mouth. The doctor can then see inside the throat on a video monitor, and therefore more easily and quickly intubate and perform the exam.
To create a groundbreaking device, the team observed over a dozen laryngoscopies, bronchoscopies, and intubations. They noticed that many anesthesiologists relied on fixed equipment during the procedures. For example, some had grown accustomed to looking over their shoulder—away from the patient—to view the monitor.
During interviews, physicians and residents expressed difficulties with the current technology, sharing that the small screens aren’t conducive to team viewing or student instruction, and they must often use a separate computer to record patient notes. Others reported frustrating workflows, needing to constantly divert their attention and move their hands between the patient, prep tray, monitor, sterile bin, and disposal bin. These inconveniences and inefficiencies are not only cumbersome, but can increase the risk of infection and fatality.
The team relied on these insights to start designing a next-generation product. Through sketching, storyboarding, and building in real-time alongside doctors and clinic staff, designers quickly teased out additional findings and validated concepts.
This work led to the creation of the GlideScope Core: an all-in-one, portable product system that supports both video laryngoscopies and bronchoscopies. The base of the device—a wheeled cart—has built-in storage units for tools and a disposal bin, offering more mobility and accessibility for medical professionals.

The product’s tagline, “see more, do more,” points to the improvements made to the video system, including a large 10- or 15-inch high-definition touch screen monitor. Displayed images can be enlarged and rotated 180 degrees for improved visualization, team viewing, resident training, and classroom demonstration. To help doctors keep both the patient and monitor in their line of sight, the detachable screen can be secured to an adjustable arm that can be positioned directly over the patient. Additionally, the monitor’s patient annotation feature gives staff access to more of what they need in one device.
The product has been especially valuable for providers and patients during the coronavirus pandemic, when it’s imperative for doctors to keep their distance from patients’ mouths and noses. The GlideScope Core’s movable, detachable, and large, high-definition monitor makes it easier to take this precaution and intubate more quickly. This usefulness is reflected in the 5-fold increase in cart-based device orders for Verathon. While most providers typically order anywhere from 1 to 20 devices for airway procedures, one hospital system requested 600 GlideScope Core systems to meet current intubation demands.
Through modern technology and a focus on human needs, the GlideScope Core supports improved patient outcomes and creates a more seamless operating experience for medical professionals. The product underlines the key but often overlooked fact that often, in order to ensure the healthcare industry is patient-centered, medical device manufacturers and hospitals must also focus on improving the experience of providers.


Transforming a community hospital from the inside out
Designing for caregivers to create world-class care for patients.
Healthcare is notoriously difficult to change. Rules and regulations present complex barriers; health practitioners and organizations often have entrenched ways of working; and lives are literally on the line. Despite these significant barriers, the leaders at Newton-Wellesley Hospital (NWH) boldly committed to improving patient care through a new approach.
NWH is a community teaching medical center in Newton, Massachusetts. Because of its proximity to Boston—a city known for world-class healthcare institutions—NWH needed to differentiate itself. The hospital's new president, Dr. Michael R. Jaff, reached out to IDEO for help: "I knew that providing an unparalleled patient experience would set us apart from our competitors." But, Dr. Jaff’s particular approach was unique. He and Dr. Jodi Larson, Chief Quality and Experience Offer (CQXO), saw an opportunity to drive better patient outcomes through cultural and organizational change.

IDEO partnered with NWH for a year of work together, kicked off by defining the vision for NWH and ramping up to in-hospital prototyping and the launch of several new initiatives. A pilot project with the Women's Imaging Center (WIC) focused on solving departmental and employee challenges to provide a better experience for women getting mammograms.

Fueled by this potential, the WIC team redesigned key work processes and created a set of experience principles that inspired new service prototypes. Guided by principles like "Express mutual gratitude" and "Honor every voice," the WIC team tested ways for patients to acknowledge outstanding caregivers and created tools to gather better feedback from patients and adjust quickly. The experience of designing better services inspired the WIC staff to become ambassadors for the power of design thinking across other departments inside NWH and they continue to build on the work through new prototypes and experiments.
Next, IDEO partnered with NWH’s ambulatory surgery group. They took an analogous research trip to Boston’s Logan airport to see with fresh eyes how other high-touch customer service organizations provide customer value in a complex setting. Insights from the trip were applied through live prototyping of several new experiences inside the hospital, including a pre-surgery visit summary and pre-operation care carts.

One of the most impactful prototypes was Care Modes, a tool to empower the NWH care teams to tailor their approach and interaction with patients based on the patients’ unique needs, beyond simply their clinical requirements. Four distinct approaches were identified for engaging a patient before surgery—Explain, Partner, Embrace, and Encourage. The Care Mode is determined based on the provider's understanding of a patient's degree of self-advocacy and their confidence that surgery is the best solution to their issue.

Historically, care providers have been trained to deliver consistent care above all else. However, Care Modes powerfully acknowledge that patients respond differently to different types of care. The Care Modes empower the team—from surgeon to nurse to anesthesiologist—with a clear framework for how to interact with and provide care for patients so they feel most comfortable. Based on the prototype's success with patients and impact on the staff culture, NWH has incorporated Care Modes into EPIC, the hospital's electronic medical record.
The importance of being patient-centered has been central to the healthcare conversation over the past thirty years; but new explorations reveal that there’s far more potential for innovation and improvement when the provider experience is considered in tandem with the patient’s. Each enables the other, and design for both together yields stronger solutions and better outcomes. IDEO's work with NWH demonstrates this vital shift in user-centered healthcare—one can't design services and experiences for patients without designing for their providers’ culture as well. When these go hand-in-hand, meaningful institutional change in healthcare is more than possible.


Helping a historic museum prepare for the future
Using design thinking to improve access, experience, and culture.
The mission of Chicago's Museum of Science and Industry (MSI) is to inspire children to achieve their full potential in the fields of science, technology, medicine, and engineering. Leaders at MSI realized they needed to drive day-to-day operations and maintain historic exhibits while focusing on where to go next. To prepare for its next decade, MSI worked with IDEO to create a new business focus to support the museum’s aspirations for the future. The plan would allow MSI to respond to the demands of the marketplace and raise the funds to get there.
At the beginning of the project, MSI laid out its priorities: improve equity of access, enhance the museum experience, and create a more innovative culture. To dive into each of these areas, IDEO conducted immersive research with families, teachers, donors, and staff.

IDEO brought together 60-plus donors, board members, and staff members to engage in a design thinking exercise to unpack and address the tensions that come with choosing a new strategic trajectory. Instead of having a standard ballot vote, the IDEO team encouraged participants to get up and vote with their feet, helping the group of stakeholders physically align on the museum’s new direction.
At the end of the research and prototyping phases, MSI and IDEO developed a vision called Provoking Minds, Inspiring Futures, creating six detailed design briefs to equip MSI teams to bring initiatives forward. The briefs laid out everything from recommending a new Department of Next, dedicated to ensuring that the future is at the heart of everything the MSI does, to MSI Runway, a place for experimental, small-scale, and limited-run exhibits. The ideas were brought to life with an interactive exhibit, built quickly with everyday materials like plywood and circuit boards, to demonstrate that creating an inspiring vision of the future doesn’t have to be an expensive endeavor.
The team also focused on insights that highlighted accessibility issues some families brought up early in the design process—travel across the city via public transportation can be lengthy, and the cost of admission to special exhibits could be prohibitive. To address these concerns, IDEO came up with the Mission Accessible brief: Inspired by casino buses, MSI bus stop signs would be posted in various low-income neighborhoods, signaling a way to take kids and families to the museum for free.

During the project, the IDEO team identified some low-hanging fruit, like rethinking ticketing and improving the student field trip experience, and helped the museum create cross-functional innovation teams to apply design thinking skills towards solving those challenges. They researched, brainstormed, and pushed forward new concepts, meeting weekly to share and build on ideas. At the end of the four week period, each team prototyped their ideas—new outdoor programming, software tools for visiting tour groups—and pitched them to each other.
In 2019, two years after the conclusion of the project, the collaboration paid off. With its new strategic plan and additional resources, the museum has renewed its commitment to serving youth, and MSI’s team, including donors, are aligned on a collective vision of the future. MSI raised more funds than in any previous year, including the single largest gift in its history—$125 million, nearly half of its $380 million fundraising goal—that will help ensure the years ahead.


Beat-Boxing Our Way to a Better World
One way to give refugees a voice
It all started when we partnered with the American Refugee Committee (ARC), an organization that provides services to 3.5 million refugees across Africa and the Middle East. We teamed up to imagine a future where refugees have a voice in shaping the services provided to them, and are able to hold humanitarian organizations accountable for the services they receive.
The result was Kuja Kuja, a platform that has the potential to fundamentally shift the way humanitarian organizations serve refugees.

In a deep and humbling way, the friendships we made during those two weeks reminded us of how refugees, who are typically lumped together as a faceless, nameless group of displaced people, are smart, educated, optimistic human beings who've been brutally ripped from their homes by war and the vagaries of governance.
There are more than 65 million refugees and displaced people around the world today. That’s nearly 1 out of every 100 people on the planet. These people are no different from you or me in their need for safety, shelter, work—the most basic human rights.
On our last day in the settlement, at the end of the workday, we went to play ping-pong at a community center.
It was raining on and off, and there were a lot of people packed inside. There was a group of Congolese refugees breakdancing and practicing for a hip-hop dance competition. One of the dudes was beatboxing while his friends pop-n-locked and windmilled.
I started to beatbox with him and soon we had all kinds of people busting into our jam-circle to throw down raps in multiple languages—French, Swahili. It was the best example of getting close to the people you're designing for I could ever imagine.
I will certainly never forget that day. I glimpsed a world in which, if we were were all trapped in a room together, and broke out in song and dance, there would be no more war, no more displacement, no more refugees.
Back in August 2017, I was embedded at the Nakivale refugee settlement in Southwest Uganda for two weeks. I was there with Nathalie Collins from IDEO.org, to build a web application that collects refugee feedback and then visualizes that data in meaningful ways. Those two weeks could have been two months—Nathalie and I developed deep friendships with many of the refugees on the team. I'd been vocal-jamming earlier in the week with a pentalingual Burundian refugee and Kuja Kuja team member, and someone started videotaping...

The Designer’s Toolkit for Tackling Tough Problems
Bite-sized tools to spark creativity in the face of daunting design challenges
The constraints and complexity of working on those challenges can be overwhelming. To help us all push through, we developed a few simple tools to keep in your back pocket. Here are three of them, drawn from the Design Kit Travel Pack, a new deck of bite-sized exercises we’re working on to help social impact organizations and busy professionals unlock their own creative genius.

1. Write a postcard from the future
In the social sector, the language we use is all about fixing things that are broken. We talk about reducing unintended pregnancies or increasing the number of people with access to financial services. It’s important work, but not always framed in the most inspiring way. Before you kick off a new project or initiative, take the time to paint a more audacious vision. Close your eyes, picture a time 20 years in the future, and imagine your work has been a resounding success. What does the world look like? How are people’s lives different? Be as specific as possible. Then, write a newspaper headline for a New York Times story capturing this impact and sketch an image to go with it. Share your vision with your team, and use it as your North Star as the creative process becomes more murky and new challenges emerge.

2. Take an inspiration tour
When we’re designing in conditions with tight constraints—like a tightly-packed informal settlement outside of Nairobi or a rural village in India—it’s vital that we understand and work within the local context. But to unlock fresh solutions, we also need to look elsewhere and gather inspiration from other (sometimes unlikely) places. To design your own inspiration tour, start by naming three adjectives you want your product, service, or experience to embody. For example, maybe we want the new health clinic we’re designing to feel safe, fun, and community-affirming. Then, quickly brainstorm 10 places, businesses, or brands that do this well in your city. (Examples: a sports team’s locker room, a Bible study group, a Weight Watchers meeting, a co-working space, or a comic convention.) Choose a couple of places that are most interesting to you and take a day to visit them in person. Note down your observations as you go, take photos, and look for clues about what might inspire your project. By casting off the constraints of the context you’re working in, you’ll start to see new possibilities.

3. Make time for retail therapy
When you’re feeling stuck and not sure which direction your project should take, try getting tangible (even before you might feel ready). One fun way to do this is a trip to your local hardware or container store. Give everyone on your team 15 minutes to explore the store and create their own scrappy set-up of the product or service you’re designing. Then, one at a time, walk each other through the prototypes you created. Forcing yourselves to use materials available in the store and using a timer helps narrow the scope of what you’re working on from a big, nebulous problem to an immediate and concrete one, often unlocking fresh thinking in the process.

Here are nine travel experiences we’d love to redesign:
GETTING THROUGH AIRPORT SECURITY
Taking our shoes off to go through security? Gross. But the most stressful moment for me is when my stuff comes out the other end of the scanner and I have to grab it in a rush because someone else's stuff is right behind and my toddler is running in the other direction. Then we waddle over to a corner with all our belongings splayed out and try to make sense of our luggage again. There has to be a better way!
Then there’s all the time wasted. Could there be a way to check wait times for security lines—similar to a traffic report—so I know how far in advance I need to arrive at the airport? IDEO designers are already working with the staff of a major Canadian airport to make their screening process smoother, faster, and less unpleasant. Let’s keep the good ideas rolling!

THE PLANE BOARDING SHUFFLE
I’ve noticed a weird social phenomenon: Everyone gets up when the first people start boarding a plane, even if they have three boarding zones ahead of them. The zones are arbitrarily assigned based on when passengers checked in and how much extra money they wanted to pay—a legacy system based on someone's obscure definition of loyalty and status, plus assertiveness. That means that if someone in Zone 2 gets into an aisle seat, but I'm in Zone 4 and have the window, everyone has to get up, get out, shuffle down the aisle, let me in, and then sit down again. It's entirely inefficient. Also, if I have no overhead carry on and can breeze off the plane, facilitate that. I've never met anyone who actually likes this system. Airlines are leaving money on the runway—literally.

FLYING ECONOMY CLASS
I used to work for Emirates, Singapore Airlines, and Qantas, so I have a vision for the ideal plane cabin. In economy class, you get shoved into an uncomfortable seat next to a guy who hogs the armrest. This is painful for people like me who usually take long-haul flights. (I fly 15 hours from Dubai to Sydney twice a year to see family.)
Why not reconfigure economy class with bunks for passengers on flights longer than four hours, so that people can actually relax or sleep? Do away with the overhead lockers, increase the checked baggage allowance, and allow passengers only one small carry-on unless they’re traveling with kids or a baby. Designate time slots for people to get meals, which they can order beforehand or through the in-flight entertainment system, letting flight attendants better organize their work. You could also automate the system of delivering meals to passengers instead of pushing a trolley down the aisle, which blocks me from getting to the bathroom while an indecisive passenger is trying to figure out if he wants a Sprite or a Coke.
While we’re at it, have a kids' section where they can play without annoying everyone else, and include something like the in-flight nanny service Emirates already offers. Have a nursing area where mums can feed their babies without people bumping past them, or change their babies without having to juggle the kid, a dirty diaper, a fold-down table, and a diaper bag in a bathroom without enough room to swing a hamster, let alone a cat.
Check out this image of a crew rest area on an airplane; the stewards have room to lie down and snooze, while not being dear on space. With just a little imagination, you could make these into seats that fold flat and include easy-to-clean access, since cleaning the airplane between flights takes longer than anything else. The specs of an Airbus A380 would make this setup totally possible!

RIDING THE CROSS-COUNTRY RAILS
Two years ago, I took a long train trip from Chicago to New Orleans with my wife. She’s Danish. I’m Turkish. We have a lot of experience with train travel in Europe. It was shocking to see how sad train conditions are in the U.S. How might we redesign the experience of the Great American Railroad? We can utilize all our design chops—from data to spatial to UX—to do so. I’d love to take the lessons I’ve learned from IDEO’s relationship with Ford and apply them to train travel to make an impact on a more economically and geographically diverse community.

TRAVELING WITH A KID (AND A CAR SEAT)
It’s so annoying to travel with a car seat. I avoid it at all costs, and then end up trying to rent or borrow one, which is never convenient. It's also a pain when I want to do something like take the train into the city and then take a taxi to a part of town that’s too far to walk to with a toddler.
How about a packable, maybe inflatable, car seat? Can you imagine how great it would be if you had a small, portable car seat you could just throw in your bag?

BOOKING A NATURE GETAWAY
I want to redesign the way we book campsites. The current system is really challenging to navigate, particularly for outdoors neophytes. It's opaque and cumbersome, and there's no way to release sites for last-minute booking. I'd love to design ways to make the process simple, intuitive, and experiential.
Shouldn't a booking site feel welcoming and fun? Couldn't it show you what kind of hike to expect on the way to your campsite and what the views are like? If it’s a car camping site, how noisy is it? How about real-time weather so you know what to pack, or a map showing how close your site is to amenities like bathrooms or a store? What if it included ratings from other campers? Let's make our parks and forests more accessible, especially to those who aren't experienced outdoors people.

DODGING TOURISTS IN THE BIG APPLE
New York City sees more than 60 million visitors a year, and locals know that with tourists come clogged sidewalks, long lines, and dozens of confused subway riders. As a New Yorker, I’m also struck by how seldom visitors tend to go outside the typical destinations—a.k.a. Midtown and the usual suspects. I love how distinctive each neighborhood of New York City is, and the unique culture, food, and experience each offers.
I know we can do a better job of moving people around the city so that visitors have a better experience. I’m inspired by this initiative the city of Amsterdam is using to distribute, rather than limit, the flows of tourists. Amsterdam encourages tourists to visit outlying areas and see popular attractions at off-peak times. Why can’t we? IDEO worked with the TODAY show to design a seamless experience for the street audience at Rockefeller Center; we can put some of those same techniques to work at a larger scale.

NAVIGATING THE URBAN LANDSCAPE
I would love to see a well-designed urban infrastructure that allows for driverless mobility, makes cycling and walking safe alternatives, and accounts for a broad span of ages and physical limitations.
Right now, multimodal transportation is not synced. Urban dwellers need various types of transit to get around, from trains to buses, but none of them talk to one another. There's so much focus on driverless cars, which is great, but we also need to seize the opportunity to encourage more active mobility by creating more car-free zones for safer walking and biking, for example. Both protected bike lanes and central bikeways would encourage more safe cycling in cities.
In general, street signage is not all that helpful or appealing. Might there be a way to not just wayfind, but also add delight? Signs could provide more than just basic information, such as interesting historical tidbits about the city, the number of steps between signs, or digital messages on sidewalks or in other unexpected places. People could also track their city journeys via an app on their phones to show where they've been, and get recommendations for where to go next; their urban journeys would become living maps.

PAYING FOR PARKING DOWNTOWN
Parking lot pay machines are poorly designed. They’re often covered with lists of instructions and homemade signs taped on by the operators and owners trying to show customers what to do and in what sequence. The machines need to accept credit cards—with a stripe in one exact orientation—bills, coins, and of course parking tickets with a magnetic stripe in one orientation, too.
I like how the parking garage at San Francisco International Airport allows you to use FasTrak to avoid a paper ticket. Maybe ticket machines could use Apple Pay or another pay-by-phone service to avoid credit cards and cash, or better yet, use pay-by-phone services in your car on the way in and out of the lot so you can avoid the ticket machines entirely.

IDEO already does forward-thinking transportation design with partners like Ford, Lincoln, Lufthansa, and major airports. But transportation is a sprawling industry, and there’s always plenty more to do.

I started my career in editorial and digital design, shaping cross-platform experiences and identity systems in publishing and agency environments. That foundation still shapes how I work today: I’m detail-oriented, collaborative, and overreliant on the Oxford comma. I hold a BFA in Visual Communication from the School of the Art Institute of Chicago.

For 25 years, I've asked the same question: Who does this design exclude—and what are we going to do about it?
My work spans human-centered strategy, inclusive design, and qualitative research, with clients ranging from Microsoft, Google, and Verizon to the National Science Foundation, and San Francisco Unified School District.
Before IDEO, I taught elementary school in East Palo Alto and spent a decade doing design work with social service organizations—where I learned firsthand what it costs people when systems are built without them in mind.
I am currently writing User Error, a nonfiction book about digital access and the design decisions behind it. I live in Oakland, California.

In over 15 years in IDEO Shanghai, I have worked on projects that use design to elevate the quality of the experience of healthcare products and services, streamline processes that increase productivity, create spaces and programs that promote and enable inclusive communities, and build next generation mobility solutions that are planet-positive.
Before joining IDEO, I worked at Philips Electronics and the Electrolux Group in Italy, the Netherlands, and Singapore on a number of breakthrough commercial products. I am a member of the Young President Organization in Shanghai.

So she built Cue, a personal AI that lives inside iMessage and group chats, to go where the other tools haven't.
